How to modify exercise when your knees hurt

Knee discomfort can affect walking, squatting, stairs, and many common workouts. The sensation alone does not identify the cause, so the safest response is to reduce irritation and pay attention to how the knee behaves.
Describe the symptom before changing the workout
Notice where the discomfort appears, which movement brings it on, and whether it changes as you continue. Also note swelling, warmth, catching, instability, or a recent fall or twist. These details can help a healthcare professional understand the problem. They should not be used to diagnose yourself from a checklist.
A mild ache that stays stable may respond differently from sharp pain that increases with every repetition. Use a conservative approach when the symptom is new. Stop the specific movement, walk around gently if comfortable, and reassess. Do not repeatedly perform deep squats or stairs just to confirm that the knee still hurts.
Adjust range, speed, and support
Reducing the depth of a squat or the height of a step may make the movement more comfortable. Slowing down can improve control and show where discomfort begins. A stable counter, railing, or chair back can provide balance support, but it should not be used to pull yourself through significant pain.
Keep your foot fully supported and allow the knee to follow a comfortable path rather than forcing a rigid position. Stance width and toe angle can vary. If you need to twist, collapse inward suddenly, or shift nearly all your weight to the other side, the current version is not working well. Choose a simpler variation.
Train around the aggravating movement
A painful exercise does not always require stopping all activity. You may be able to perform upper-body training, gentle cycling, hip exercises, or a shorter walk if those options remain comfortable. Select activities based on your response during and after the session. A movement that feels fine initially but causes a clear symptom increase later may still need adjustment.
Avoid using hard exercise to test whether the knee has recovered. Reintroduce the aggravating pattern gradually, beginning with a smaller range or lower resistance. Track what you did and how the knee responded over the following day. This creates more useful information than changing several exercises and loads at once.
Know when professional assessment is appropriate
Arrange an in-person assessment if discomfort persists, keeps returning, or interferes with walking, sleep, work, or normal household tasks. Swelling, locking, repeated giving way, reduced range of motion, or unexplained weakness also deserve evaluation. A remote trainer can observe movement and offer general modifications but cannot examine the joint.
Seek prompt care if you cannot bear weight, the knee looks deformed, swelling develops rapidly after an injury, or a significant fall or collision occurred. A painful pop followed by loss of function should also be assessed. Avoid driving yourself if the affected leg cannot control the vehicle safely.
Return with gradual loading
When normal daily movement is comfortable and you have appropriate guidance, rebuild exercise in small steps. Start with a manageable range and low resistance. Increase repetitions or range before adding a large amount of load. Keep other parts of the program stable so you can identify how the knee responds.
Stop and seek urgent medical help if knee symptoms occur with chest pressure, fainting, severe shortness of breath, or sudden neurological changes, because those are not routine exercise complaints. If the knee becomes hot, markedly swollen, or painful along with fever or feeling seriously unwell, obtain prompt medical evaluation rather than attempting another workout.
