Telehealth for children: What parents should know

Virtual pediatric care can assess many mild concerns, but the child must be present and some symptoms require hands-on evaluation.

Telehealth can make it easier to discuss a child's symptoms from a familiar setting. It works best when a parent or caregiver can provide a clear history, help with the camera, and recognize when the child needs in-person care.

Make sure the child is part of the visit

The provider needs to see and interact with the child when possible. Do not schedule a visit about a child who is elsewhere unless the service specifically allows a caregiver at that location to participate. The provider may need to observe breathing, alertness, movement, speech, skin color, and how the child responds to you.

Choose a time when the child is likely to be awake, but do not delay care for concerning symptoms. Have a second adult help if you need to hold the device or comfort a young child. Explain the visit in simple language and avoid forcing a frightened child through an exam maneuver that could cause injury.

Prepare a clear symptom history

Write down when symptoms began, whether they are improving, and how they affect drinking, eating, sleep, play, and bathroom habits. Mention fever, cough, breathing changes, vomiting, diarrhea, rash, pain, or unusual sleepiness. For babies, include feeding changes and the number of wet diapers compared with normal.

Have the child's medication list, allergies, medical conditions, and recent weight available if known. Tell the provider about recent illnesses, daycare or school exposures, travel, injuries, and household contacts. Do not give a medication based only on an adult dose or another child's instructions.

Set up for a guided observation

Use a well-lit room with space for the child to sit, stand, or walk if appropriate. The provider may ask to see the throat, ears from the outside, a rash, or the way the chest moves during breathing. A flashlight and a stable camera can help, but ordinary home equipment cannot replace a full pediatric examination.

The provider may ask you to count breaths, check a temperature, or gently press on an area while describing the child's response. Follow only instructions you understand and can perform safely. A home device result is one piece of information and should not override visible breathing trouble, poor responsiveness, or other serious symptoms.

Expect some concerns to need an office visit

Ear pain often requires an otoscope examination, and significant abdominal pain may require hands-on assessment. Breathing concerns may require lung sounds and clinical oxygen measurement. Urinary symptoms, sore throat, or persistent fever may require sample collection. The virtual provider can help identify the appropriate location and urgency.

Ask what to monitor after the visit and when the child should be rechecked. Children can change quickly, so the plan should include clear warning signs. If the child looks worse than during the appointment, seek further care even if the original plan was home monitoring.

Recognize pediatric emergencies

Call 911 for severe breathing difficulty, bluish or gray lips, a seizure, loss of consciousness, choking, serious injury, or a child who cannot be awakened normally. Sudden weakness, severe allergic symptoms, or rapidly increasing swelling of the face or tongue also requires emergency care.

Seek prompt in-person care for breathing that is fast or labored, signs of dehydration, a stiff neck, severe pain, persistent vomiting, a concerning rash with illness, or unusual lethargy. Fever in a young infant requires immediate professional guidance. Trust visible changes in the child and do not wait for another video visit when they appear seriously ill.
