How telehealth supports ongoing primary care

Telehealth is often associated with short-term illness, but it can also support an ongoing relationship with primary care. Its value depends on good records, follow-through, and knowing when a physical examination is necessary.

Use virtual visits for focused follow-up

A virtual visit may be useful for reviewing symptoms, discussing test results, checking progress after a prior visit, or addressing medication questions. It can also support conversations about sleep, stress, preventive care, and health goals. The provider will decide whether the issue can be handled remotely based on the information available.

Choose one or two priorities for the appointment and write down what has changed since your last visit. Bring recent readings or records if they are relevant, but include when and how they were obtained. Home measurements can add context without replacing clinical measurements or a physical examination.

Keep your health information connected

Tell the virtual provider about your regular primary care clinician, specialists, recent urgent care visits, and outside test results. Maintain an updated medication and allergy list. Fragmented information can lead to repeated testing, missed interactions, or plans that do not account for your full health history.

Ask for a visit summary and share it with the rest of your healthcare team when appropriate. Confirm who is responsible for reviewing tests, renewing ongoing medicines, and arranging follow-up. A referral or test order is only useful if someone tracks the result and explains what it means.

Understand the role of remote monitoring

Some people use home devices to track blood pressure, blood sugar, weight, temperature, or other measurements. A provider can explain which readings are relevant and how often they should be collected. More data is not always better, especially if the device is inaccurate or the measurements are taken inconsistently.

Bring the device or its instructions to an in-person appointment periodically if the healthcare team recommends checking its accuracy. Report symptoms along with the numbers. A reassuring reading should not delay care for chest pain, severe shortness of breath, fainting, new confusion, or another emergency.

Plan for examinations and preventive care

Primary care sometimes requires listening to the heart and lungs, examining the skin, checking pulses, or performing other hands-on assessments. Vaccinations, sample collection, imaging, and many screening procedures also take place in person. Telehealth works best as one part of care rather than a complete replacement for every office visit.

Ask the provider which preventive services may be due based on your age, history, and individual risks. Recommendations can change, and not every screening applies to every person. The virtual visit can start the conversation and arrange next steps without promising that all preventive needs can be completed online.

Escalate new or worsening symptoms

Contact the healthcare team if symptoms change after a virtual visit, a new medication causes a concerning reaction, or you cannot complete the recommended testing. An in-person appointment may become necessary even when the original concern was appropriate for telehealth. Keep the written instructions so you know whom to contact.

Call 911 for severe trouble breathing, chest pain or pressure, signs of stroke, loss of consciousness, uncontrolled bleeding, or another sudden emergency. Routine primary care messages and scheduled video visits are not monitored like emergency services. Use urgent or emergency care whenever delay could be dangerous.
