When to see an online doctor for a cough
A virtual visit can assess many uncomplicated coughs, but breathing trouble, chest pain, or coughing blood needs prompt in-person care.

A cough may accompany a respiratory illness, allergies, airway irritation, reflux, or another health problem. The sound of the cough matters less than the full pattern, including breathing, duration, exposures, and other symptoms.
Describe the cough in context
Tell the provider when the cough began and whether it is dry or brings up mucus. Describe how often it occurs, whether it is worse at night or with activity, and whether it interrupts sleep. Mention fever, congestion, sore throat, wheezing, fatigue, heartburn, vomiting, or chest discomfort.
Explain whether you were recently around someone with a respiratory illness or exposed to smoke, dust, fumes, mold, or a new animal. Share any history of asthma, lung disease, heart disease, reflux, or seasonal allergies. The provider will also want to know whether a new medication was started before the cough appeared.
Prepare for a breathing assessment
Sit in a quiet room where the provider can hear you speak and breathe. They may ask you to take a breath, cough, walk briefly, or count while speaking. A home temperature or oxygen reading may provide context if you already have reliable equipment, but a normal device reading does not overrule severe symptoms.
Telehealth cannot fully replace listening to the lungs, measuring vital signs with clinical equipment, or obtaining imaging. Background noise and poor microphone quality can also distort breathing sounds. If the provider is concerned about pneumonia, an asthma flare, or another condition requiring examination, you may be directed to in-person care.
Questions about testing and treatment
Depending on the symptom pattern, the provider may discuss respiratory testing, an in-person lung examination, or imaging. They may also review general comfort measures and whether nonprescription products are safe with your health conditions and current medications. Not every cough requires an antibiotic or another prescription.
Do not use leftover antibiotics, share inhalers, or take another person's cough medicine. Some products contain overlapping ingredients or may be unsafe with certain conditions. If a prescription is considered, the provider should review allergies, pregnancy status, age, medication interactions, and the limits of the virtual assessment.
Reduce spread while symptoms are active
If the cough may be part of a contagious illness, limit close contact and improve airflow in shared spaces. Follow current public health guidance about staying home, masking, and testing. Wash your hands after coughing or handling tissues, and avoid sharing drinks, utensils, or personal items.
Tell the provider if you live with someone at higher risk of serious illness. They can help you think through testing and follow-up without promising that household spread can be prevented. If symptoms change after the visit, update the healthcare team rather than assuming the original assessment still applies.
Know when a cough is urgent
Call 911 for severe trouble breathing, bluish or gray lips, new confusion, fainting, or chest pain or pressure that may signal an emergency. Seek prompt in-person care if you are coughing up blood, cannot speak comfortably because of breathlessness, or feel that your airway is blocked.
An in-person examination is also important when a cough is steadily worsening, lasts longer than expected, returns repeatedly, or occurs with dehydration, persistent fever, significant weakness, or shortness of breath at rest. Infants, older adults, pregnant people, and those with heart, lung, or immune conditions may need earlier assessment.
